2007-2008 Junior Team Application

We hereby make application for scheduling with the USA League. We have read the rules and regulations and agree to abide by them or any modifications and additions thereto during the season. By clicking "accept", we hereby agree that USA has disciplinary authority over our Club and this team, for any infractions of the rules of soccer, relating to any play under the jurisdiction of the USA League.
